Office Policies

Cancellations:

as a courtesy to other clients trying to schedule appointments, at least 24-HOURS NOTICE  is required for cancellations/reschedules. Cancellation with less than 24-hour’s notice or a “no-show” will result in being charged $50 for the missed appointment. If you have a stored credit card on file this card will be charged the cost of the fee. Otherwise an invoice will be submitted to you to reflect these charges and payment is due before your next visit.

Canceled/returned checks:

All canceled/returned checks for any services/sales will carry a $20.00 fee, which is due before your next visit, in addition to any balance due for services rendered during the visit corresponding to the canceled/returned check.

Tardiness:

Appointment times are as scheduled and cannot extend beyond the stated time to accommodate late arrivals. I will gladly see you, but cannot cut into the next client’s time slot.

Attire:

You will need to wear/bring comfortable (athletic) clothing to accommodate proper examination and treatment including, but not limited to manual therapy, stretching and rehabilitative exercise. Please dress accordingly (and cleanly).

Pricing:

Prices are non negotiable and subject to change without notice.

Conduct:

You understand that any illicit or sexually suggestive remarks or advances made will result in immediate termination of the session, and you will be liable for payment of the scheduled appointment. You also understand that you will no longer be able to schedule any further sessions. You understand that authorities may be contacted if the practitioner deems necessary.

Confidentiality:

You understand that you are entitled to confidentiality with certain exceptions in which reporting may be legally required.

Consent for treatment:

As part of the analysis, examination, and treatment, you are consenting to the following procedures including but not limited to: range of motion assessment, manual muscle testing, palpation, posture analysis, vital signs, neurologic examination, exercises, cranial therapy, amino neuro frequency therapy, emotional therapy techniques, emotional trauma techniques, functional medicine coaching, functional neurology, or manual therapy (use of hands or instruments for treatment of soft tissue: i.e. massage, myofascial release, active stretching, etc.). These services will be applied at the discretion of Evolve Within, LLC (“EW”) based on examination findings.

If you experience pain or discomfort during your session, you will immediately inform EW so that the treatment can be adjusted to your comfort. You further understand that no matter the form of therapy you receive from EW, such should not be construed as a substitute for medical examination, diagnosis, medical advice or treatment and that you should see a physician, chiropractor, or other qualified medical specialists for any mental, physical or health ailment of which you are aware. You understand that the session effects have a minimum 3-day period of adaptation where you will feel any number of sensations as your body integrates the work. You understand that bodywork practitioners are not qualified to perform spinal or skeletal adjustments, diagnose, prescribe, or treat any physical or mental illness and that nothing said in the course of the session given should be construed as such. Because bodywork should not be performed under certain medical conditions, you affirm that you have stated all known medical conditions and answered all questions honestly. You agree to keep EW updated as to any changes in your medical profile and understand that there shall be no liability on EW’s part should you fail to do so. You understand that no specific results are guaranteed or promised. Nothing discussed during your treatment is meant for medical advice or is to be taken as any sort of medical diagnosis.
